Incident Reports
An incident can be any accident that occurred during the performance of laboratory work activities that resulted in or could have led to injury or damage to property.
This can include incidental spills, emergency spills, fires and injuries.
A “near miss” is an incident not resulting in personal harm or property damage, but could have under slightly different circumstances.
- Always report “near misses” just as you would an incident that causes injury or harm to property
- Doing so will prevent future, more serious incidents from happening

Incident Reports must be submitted to Research Safety within 5 business days of the incident.
For non-life-threatening injuries, follow the instructions on the Worker’s Compensation page on the Risk Management website.
