Incident Reports

New: Effective 1/2/25 we have switched to using the Incident Reporting system through Risk Management. You do not need to submit a separate report to Research Safety. Select the Report an Incident link below to begin.

An incident is any accident that occurred during the performance of laboratory work activities that resulted in or could have resulted in injury or property damage. This includes but is not limited to incidental spills, fires, and hazardous material releases.

A near miss is an incident that did not result in injury or property damage, but could have under slightly different circumstances.

Report an Incident (netID log-in required)

  • After logging in, select the Report an Incident Button under "Injuries or Near Misses". You will see the following options:

  • For injuries, select either Injured Employee or Injured Non-Employee. Graduate students should select Injured Non-Employee.
  • For incidents without injuries, select Near Misses.  Always report a near miss just as you would an incident that causes injury or property damage. Doing so may help prevent future, more serious incidents from happening.

Incident Reports must be submitted within 5 business days of the incident.

For non-life-threatening injuries, follow the instructions on the Worker’s Compensation page on the Risk Management website.
